Neurospine and Pain of Key West’s Patient Information

Our practice is dedicated to providing individualized, compassionate spine and back care to all patients suffering from acute or chronic pain.

Our highly trained staff and physicians utilize a multidisciplinary approach which gives our patients the best chance of recovering from pain. We have found that combining the newest advances in medicine and technology with an holistic approach helps our patients return to their important daily activities with an improved quality of life.

In order to provide patients with the best possible care, all of our providers remain trained on all of the newest procedures and latest developments in the world of neurology, specifically for pain management.

Office Visit

Please bring the following items with you to each appointment:

  • Photo ID
  • Insurance card
  • List of medications you are currently taking
  • List of questions or concerns to discuss
  • Co-pay or payment for services rendered

New Patients

If you are a new patient, you should bring any previous records, labs, x-rays, images or tests that you may have had in the past to your appointment. If you are seeing one of our physicians regarding pain management, understand that it is at the discretion of the physician as to whether any prescriptions will be written for on the first visit. Also, you should complete the New Patient Paperwork prior to arriving at our office for your first appointment.

Please arrive approximately 20 minutes prior to your first scheduled appointment for the registration process, during which time you will be asked to complete new patient forms.

Established Patients

If you are an established patient and have not been in for a visit in a year or if there have been any changes in your address or insurance since you were last seen, you should complete the Demographics Form. If there have been any changes in your medical history, medications, allergies, etc. since your last visit, please either complete the New Patient Paperwork or log on to the Patient Portal and update.

Patient Forms

Patients can download forms from right side of the page, in Microsoft® Word format, and complete the forms on this page before you are seen in our office. Completing the forms prior to your arrival for your appointment will help us treat you in a timely manner. Please fax them to us at 305-296-2209. If you do not have access to a fax machine, please bring the forms with you when you come in for your appointment along with a picture ID and your insurance card(s).

If there is anyone, such as family or other physicians, that you would like to share your personal medical information with, please fill out a Compound Authorization for release form. We refer to this as a Family HIPAA form.
