We are always happy to see your patients. Please call the office at 305-296-2212 to schedule an appointment. We will make every effort to see your patient in a timely manner. If the next appointment is not soon enough, feel free to contact one of our physicians to have your patient seen sooner.
You can also download a consultation requisition form below that allows you to specify the indication for consultation. In addition, the form contains information that we need to set up the appointment and do the pre certification if needed by the patient’s insurance. If the patient is being referred for testing, please download and provide the patient a copy of the preparation sheet for the respective test.
